Admission procedures

Required supporting documents

Depending on your situation, you will be asked to provide different documents. In all cases, you must provide the following documents:

  • An identity document
  • The hospitalisation or admission form
  • The form for appointing a trusted support person.

If you are covered by social security:

  • Your up-to-date carte vitale
  • A certificate of entitlement

If you are a member of a mutual insurance fund :

  • Your Mutuelle membership card: make sure your contributions are up to date. In addition to the co-payment, mutual insurance companies are likely to cover the fixed daily rate, the €18 co-payment and the cost of a private room.

Remember to apply to your mutual insurance company for cover and to present the agreement when you are admitted.

If you are entitled to 100% reimbursement:

  • The protocol given to you by your GP.

If you have suffered an accident at work :

  • Voucher no. 2 of the declaration provided by your employer
  • The prior agreement issued by the medical control department in the case of a relapse .

If you are entitled to Couverture Maladie Universelle (CMU) and/or Couverture Maladie Universelle complémentaire (CMUc), or Aide Médicale d'Etat (AME):

  • Notification of entitlement to benefits on the date of treatment

For minors or protected adults:

  • Operating authorisation from the person with parental authority/legal representative

When you are admitted, you will be given a unique number specific to the establishment, which will enable us to identify you safely throughout your stay in hospital, as well as during future visits to the clinic.

Compulsory contributions linked to your stay

In accordance with current regulations, you will be asked to pay certain fees. These are not reimbursed by the Assurance Maladie, but may be covered by your mutual insurance company or supplementary health insurance. Ask your health insurance company for details. There are exemptions, particularly for people receiving complementary health insurance or state medical aid. You can find the full list of exemptions on the ameli.fr website.

Fixed hospital charge: €20 per day (including the day of discharge)

The fixed hospital charge represents your financial contribution to the accommodation and maintenance costs incurred by your hospitalisation. It is payable for each day of hospitalisation, including the day of discharge. It does not apply to outpatient stays. The amount of the fixed hospital charge is set by ministerial decree.

Fixed contribution: €24 per stay

The preservation of Social Security is everyone's business and, in principle, you contribute to each of your health expenses. The part of the cost that remains payable by you is known as the "fixed contribution" or "co-payment".

The flat-rate contribution of €24 applies to procedures costing €120 or more. You may be asked to pay a co-payment for any procedure under €120.

You may not be billed for any costs other than those corresponding to the care provided or, where applicable, to any special requirements you may have requested. The amount of these special requirements, the list of which is strictly defined by the regulations and includes in particular access to a private room, must be communicated to you before the treatment is carried out.
